Create affidavit compliant maps with plotted players within a single boundary for most needs or with multiple boundaries for combined league all-star teams.  Also, create a fully populated affidavit that you can save without needing Adobe Acrobat Professional.

To view a demo of printing an affidavit map, follow the following instructions after downloading one of the demo MS Excel spreadsheets:

1. Using the application form below, the following values are required for this demo:
   
   • Upload File: the demo xls or xlsx file that you downloaded from above
   • Map Type: District
   • Sport: <select any sport>
   • State: Washington
   • District: District 8
2. Fill out the rest of the information on the form below.
3. To print just the map, leave the "Print Affidavit & Map" checkbox unchecked on the form
4. To print the map AND the fully completed affidavit, check the "Print Affidavit & Map" checkbox and fill out the additional information.
5. Use the navigation options at the top of the form/map to perform various operations.
